We are a State Licensed 3-4 Bed Adult Family Home. We accept only female residents who are ambulatory and non-ambulatory. Staff shifts include 2 staff on from 6am - 10pm and can vary according to how many residents are present during this time frame. We also have overnight awake staff. Owner's daughter is one of the residents and has had over 30 years of operating an Adult Family Home.
We work with IRIS, Inclusa (MCO), and will accept private pay. Daily rate is determined by a functional screen that determines how much personal care will be needed.
First, start with a phone call to Diana Wiesner @715-418-9217 to see if I have an opening available. If I do and you don't have your person already on IRIS or Inclusa I can help get you in touch with either agency for the financial part.
If however your person is already enrolled with one of these agencies, then let your IRIS Consultant or CRC/Inclusa know you'd like to schedule a tour of my home to see if you feel you want to take the next step towards admission.
